To view our registration forms you need Acrobat Reader.
Once you have printed the document please
follow the directions and mail it to the address on the form.
Post-dated cheques are not accepted,
for the simple reason that your child is only
placed on a team, once the cheque has been properly deposited.

Should you not hear from us and your records show your cheque was deposited, it means that your child was registered and placed on a team. Our confirmation email might have been deleted by your spam software.
Please contact the club if this happens.

Micro (age: 4)	2006	$100
Mini (age: 6 & under)	2004 - 2005	$100
Maxi Junior (age: 8 & under)	2002 - 2003	$115
MAXI (age 12 & under )	2001 - 1998	$115

Depending on participation numbers in the various age groups, the Maxi Division will be devided into 'Maxi Minor' and 'Maxi Major'.
Maxi Division Teams will be balanced for even competition.
USC reserves the right to change starting times in the Maxi Division.
