Once you have printed the document please
follow the directions and mail it to the address on the form.
Post-dated cheques are not accepted,
for the simple reason that a child is only
placed on a team, once the cheque has been deposited.
Should you not hear from us, and your records show your cheque was deposited, it means that your child was registered and placed on a team. Our confirmation email might have been deleted by your spam software.
Please contact the club if this happens.

| Requests for refunds are accepted in writing only. A $25 administration charge will be applicable. No refunds after the first session date.

Depending on participation numbers in the various age groups, the Maxi Division will be devided into 'Maxi Minor' and 'Maxi Major'.
Maxi Division Teams will be balanced for even competition.
USC reserves the right to change starting times and / or locations for any of our Divisions.

United SC is one of the few clubs in North Toronto, accepting requests to place friends on the same team. At the same time it must be understood that this is a courtesy of the club, and NOT a right of the registrant.
